Sessions
Sessions are 45 minutes in length and take place at least weekly. Payment by check or cash is due at the time of each session, unless alternative plans have been made in advance. Fees may be reduced due to financial hardship and decisions will be made on a case by case basis, upon request.

It is your responsibility to maintain your health insurance coverage and to notify me at least four weeks in advance of any possible or pending changes in your health insurance.
If you fail to keep your insurance benefits up to date or to obtain proper authorization (if necessary) for treatment, you are responsible for payment in full for sessions that are not covered.
I am a participating provider for Oxford Health Plans, United Behavioral Health, United Health Care, OptumHealth, HIP, Medicaid, and Medicare.
I do not participate in "Managed Medicare" or health insurance plans other than the ones listed above.
If you have "out of network" benefits through a PPO plan, you may submit the bills to your insurance company to receive partial reimbursment. Please notify me if you plan to do this so that I can provide you with all of the information you will need.
Keep in mind that if you miss a session, you will be charged a "Missed Session" fee that is equal to the entire fee for the session, not just the insurance copayment.
Cancellations
You must pay for all missed sessions, except for those due to a planned vacation (yours or mine).
If you call me at least 24 hours in advance and there is a mutually convenient time to reschedule within the same calendar week, your missed session fee may be waived. Repeated cancellation may be cause for termination of treatment (and referral to another provider, upon request.)
Telephone sessions will not be scheduled to make up for a missed session.
Telephone sessions may be scheduled due to extraordinary circumstances and will be billed at the rate of an in-person session. MIssed telephone appointments will also be treated as regular missed sessions.
Cancellation of sessions must be done in person, on the telephone, or through voice mail message. E-mail and text messages are not acceptable.
